Dr. Kate Eisenberg

Primary Care Physician


  • Kate Eisenberg, MD PhD has ten years of experience in delivering primary care in addition to her training in epidemiology and board certifications in Family Medicine and Clinical Informatics. She partners with her patients to support evidence-based care decisions, taking into account patient values and preferences.

  • I view my role in primary care as informing, supporting, and advocating for patients as they navigate their medical conditions, choices, and the healthcare system. I feel it is critical for patients to have as much high quality information as possible given how complex medical care can be today.

  • I have seen in primary care how difficult it can be for patients to navigate the healthcare system. I find it fulfilling to support patients in understanding the healthcare system and their medical conditions so they can make decisions about their care that make sense to them and that they feel confident about.

  • I’m chronically experimenting to try to bake the perfect chocolate chip cookie. It’s always fun even if they don’t come out perfectly.

Dr. Jen Rubin

Primary Care Physician


  • Jen Rubin, MD, is a double board-certified internist and pediatrician. She earned her MD in 2007 from Georgetown University School of Medicine. While there, she was an honored recipient of the Michael J. Caruso Memorial Award for Humanitarian Principles in Patient Care, and was inducted into the Alpha Omega Alpha national medical honor society. Jen subsequently completed her residency in both Internal Medicine and Pediatrics in 2011 at Yale New Haven Hospital. She spent the next 12 years practicing for a large healthcare system in Connecticut. Jen has a special interest in caring for patients with chronic medical conditions. She views the practice of medicine both an honor and privilege, and is known for her tireless dedication to caring for, educating and advocating for her patients.

  • Every patient has a unique story with individual challenges, but a uniform need for top quality medical care. The ability to provide this care relies on a strong knowledge base, broad clinical experience, empathy and trust. I am ready to listen, understand and help, and am dedicated to meeting patients where they are and tailoring plans to get them where they want to be.

  • Chronic medical illnesses are more common than most people realize. Patients’ journeys are filled with unexpected twists and turns from diagnosis to management to merely navigating the day-to-day. The physical and emotional impact is significant, making the ability to accurately diagnose and manage their conditions uniquely beneficial and rewarding. The opportunity to be part of the Chronius Team, a team that understands the complexities of chronic medical conditions and is determined to help navigate the journey, is so exciting.

  • I’m chronically fascinated by cultural variations in food, not only type and flavor, but also cost and accessibility. This, combined with my husband’s contagious love for travel, has led me to pay visit to a grocery store in every country to which I have been fortunate to travel. While most people are excited to visit sites in these destinations, I cannot wait to see their grocery stores!

Dr. Stephanie Karozos

Primary Care Physician


  • Dr. Stephanie Karozos graduated from Indiana University School of Medicine in Indianapolis in 2015. She then moved to Colorado for her Family Medicine residency training at Saint Joseph Hospital in Denver, where she was elected chief resident. She completed a fellowship in Integrative Medicine at St. Anthony North Family Medicine and with the Andrew Weil Center for Integrative Medicine, and is acupuncture certified through the Helms Medical Institute. She has participated in Functional Medicine training through the Institute for Functional Medicine. Dr. Karozos is board certified in both Family Medicine and Integrative Medicine.

  • Every patient deserves to be treated as the individual person that they are, with attention paid to their specific needs and health goals. I endeavor to treat patients utilizing my backgrounds in both conventional and integrative medicine, incorporating both pharmacologic and nonpharmacologic therapies in order to provide the most comprehensive care that I possibly can.

  • As a doctor who is very passionate about providing high-quality primary care, it is so exciting to see folks with chronic conditions finally getting the time and attention that they need and deserve. I look forward to partnering with patients to help them to feel both empowered and also supported and held in their health journey.

Uche Onyeyirim

Nurse Practioner


  • Uche Onyeyirim, NP graduated with her BSN degree from Temple University in 2011. She worked as a Medical-Surgical nurse for 7 years in a fast paced hospital in Philadelphia, PA. In 2017, she graduated from LaSalle University with her MSN to become a board certified Family Nurse Practitioner with the ANCC.

  • With multiple years of experience as a family nurse practitioner, I am committed to delivering compassionate and comprehensive healthcare to individuals and families spanning all ages. My dedication to assisting others propels me to remain current with the latest medical progressions, consistently enhancing my abilities in diagnosing and addressing a diverse array of health issues.

    From regular check-ups to preventive measures and the management of chronic conditions, I embrace a patient-centric methodology to guarantee every individual receives the individualized attention necessary to attain optimal health and well-being.

  • As a telehealth family nurse practitioner, I am thrilled to contribute to Chronius' mission of alleviating the challenges faced by individuals managing complex, chronic conditions. With a deep passion for healthcare and patient empowerment, I am committed to providing comprehensive care and advocacy that empowers patients to navigate their health journeys effectively.

  • When I am not caring for my patients, I am chronically nudged by a wet nose into playing “ball” with my super sweet Mini Labradoodle.

Angela Reteguiz

Registered Nurse


  • Angela Reteguiz is a Registered Nurse with 15+ years of patient care experience. She feels strongly about patient self-advocacy and empowerment. Angela is constantly striving to improve her knowledge base and is continuously participating in educational activities to ensure she is providing the most up-to-date and comprehensive care.

  • I like to use a ‘patient driven’ approach when delivering patient care. I find it helpful to know everything about a patient and learn it directly from the patient themself. The patient should always be in the driver’s seat of their care.

  • Chronius’ mission aligns closely with my own experiences as a chronic illness patient. The healthcare system can feel impossible to navigate at times, so it feels important to be part of a solution!

  • Music is my outlet; I can play 11 instruments and sing. I am chronically seen performing on stage at community theaters and at the Barclays Center and Citi Field.

Dr. Karim Hanna

Practice Director


  • Karim Hanna, MD, is an academic physician board certified in Family Medicine and Clinical Informatics. He has served in many roles at the USF Morsani College of Medicine, most recently he is the family medicine residency program director. He is a dedicated physician and chief of Family Medicine at Tampa General Hospital. He has a keen interest in health tech, using data, AI and technology to improve patient outcomes and change how we teach.

  • His approach to care is holistic and patient-centered, reminding patients that they are in charge of their care. He focuses on the entire family unit and regularly reminds himself and his students to love the patients.

  • I am thrilled to contribute to Chronius' mission because it aligns perfectly with my own goals of improving the management of chronic disease. I want to be proactive, assisting patients before their problems are overwhelming. We should not wait till the disease process has progressed without equipping our patients. Chronius will be a game-changer.

  • I am a husband and a father first. When I am not with them, I’m involved with my church community. I am chronically training and exercising and love reading or relaxing on a vacation.

Casey Berna, LCSWA

Mental Health Therapist


  • Casey Berna*, LCSWA, received her Master’s in Social Work from Fordham University over twenty years ago. Her own challenges navigating co-occurring illnesses and chronic pain inspired her to work with patients in these communities. Casey has a passion for shining a light on the intersections of chronic illness, reproductive health, and mental health. She is a patient advocate in the endometriosis, infertility and pregnancy loss communities and collaborates with many nonprofits in these fields to improve access to care.

  • Having a chronic illness and chronic pain can have a huge impact on mental health and often can create feelings of depression and anxiety. I help patients process any grief or medical trauma that they have as well as work to reduce stress, create boundaries, and navigate relationships in their life that may be challenging. Sometimes a session can simply be a place to feel heard and validated.

  • I know firsthand how living with a chronic illness and chronic pain not only has an impact physically, but impacts every single aspect of life. Chronius recognizes this and is committed to help reduce the stress and burden that patients experience through a multidisciplinary approach.

  • As someone who loves Italian food and baked goods, I am chronically on the lookout for gluten free items and recipe hacks that taste as good as the gluten filled ones and am happy to share my favorites!

Sara Graff, LCSW

Mental Health Therapist


  • Sara Graff*, LCSW is a licensed therapist. She has been in private practice for over 15 years and has experience providing therapy across the age spectrum. After obtaining her Master in Social Work degree from Washington University in St. Louis, Sara has worked in a variety of settings including hospitals, physician offices, community mental health centers, and private practice. Currently, Sara’s practice focuses on helping people move through the anxiety that comes with life transitions such as chronic illness, aging, relationship issues, caregiver stress, or career problems.

  • I focus my therapy on combining cognitive behavior therapy (CBT), mindful based interventions, and positive psychology. I guide you down a path to learn how to hope, peace, and acceptance in your life. In our sessions, we learn how to challenge fearful thoughts, live in the present moment, and practice gratitude.

  • I understand what it is like when you feel like a hurricane has just hit you when you receive that diagnosis. I understand personally the challenges facing you when life throws that curve ball at you and the unexpected arises. I believe in the strength of a strong support team who communicates and helps you move through your treatment towards a better quality of life.

  • I chronically photograph sunsets. I love sunsets and the beauty within them. My phone is filled with pictures of sunsets of all kinds of colors!

Jennifer Shapiro, RDN

Registered Dietician


  • Jennifer Shapiro*, RDN is a Registered Dietitian-Nutritionist with a master’s degree in nutrition and exercise science. Jennifer has a broad range of experience providing nutrition therapy in hospitals, outpatient settings, and private counseling. Jennifer is passionate about connecting with people and helping clients overcome their obstacles to optimize their health and wellbeing.

  • My goal is to provide patients with evidence based nutrition education as well as empathetic counseling to address the physical, mental and emotional aspects of their health journeys. I focus on helping patients foster a healthy relationship with food as well as making long term behavior changes to enable patients to take control of their health.

  • Dealing with a chronic health condition takes a village, which includes a supportive and collaborative clinical team. I’m excited to be a part of this clinical “village” to give patients the comprehensive care they deserve.

  • I chronically go to the beach to build sand castles with my daughter!

Chris Van Haren, BCPA

Care Advocate


  • Christine Van Haren, BCPA, RPh, has a lifetime of experience helping people navigate the healthcare system - as a parent, a Pharmacist, and a Board Certified Patient Advocate. She graduated from the University of Wisconsin - Madison School of Pharmacy and has worked in both community and long term care settings. Not content to watch her patients continue getting lost in our broken system, she returned to the UW - Madison campus to attend the Center for Patient Partnerships and hone her advocacy skills. Having obtained national certification as a BCPA in 2018, she now devotes her time entirely to Patient Advocacy and mentoring other Advocates.

  • As a Patient Advocate my role is to provide my clients with the tools and resources to make informed decisions about their health care and obtain the right care for THEM. My goal is to equip and empower my clients so they can live their best life - physically, mentally, emotionally.

  • I am thrilled that Chronius is so committed to its mission to reduce the burden of living with chronic illness that they offer on-demand Advocacy services for their members. It is a privilege to be part of the Chronius team.

  • I have come to accept the fact that I am a “chronic book buyer”. I love a good old fashioned book store, have an ever-changing stack of books I have yet to read, and will never apologize for buying more. So many books; so little time!

Melissa Rheingold

Care Coordinator


  • Melissa Rheingold graduated with a degree in Radiation Therapy from the New York Methodist Center for Allied Health and spent the next 11 years working as a Registration Clerk in a busy Brooklyn Emergency Room. Her education and extensive experience in a high-pressure healthcare setting have equipped her with the skills needed for her role as a care coordinator on our team. Melissa's ability to handle stressful situations, combined with her deep understanding of patient care processes, makes her a valuable asset in ensuring our care plans are efficiently executed and patient needs are met with empathy and precision.

  • Patients deserve to have healthcare run smoothly. From appointments to aftercare , it shouldn’t feel like a burden to maintain health. As your Care Coordinator, my goal is to provide patients with a seemless, exceptional service. I wait on hold, so you don’t have to.

  • After 15 years in the Medical field, I always wanted to see more being done for patients over practices. Chronius is the first company I’ve been a part of that is patient centered and truly and honestly wants to take a stand in what we know as healthcare.

  • I'm chronically curious about strategy, which led me to earn a BlackJack Dealing Certificate from casino school. This unique skill highlights my love for learning and applying strategic thinking in all areas of life.
