  • Yes, right now, our primary care patients must be Florida-based.

    Regulations require us to only provide care in the states where our practice is established and our providers are licensed, so, currently, we can only treat patients who are physically in Florida during their Chronius visits.

    We’re planning to expand to other states in the coming year. If you’re outside of Florida, you can sign up for free access to our patient empowerment platform, and we’ll let you know when our services are available in your area.

  • Yes! Chronius Health's virtual primary care services can be accessed from anywhere in Florida with a reliable internet connection. Our telehealth platform allows you to schedule video or phone consultations with your primary care provider at your convenience, from the comfort of your own home.

  • Once you become a Chronius Health patient, you can easily self-schedule appointments through our secure platform. Simply click the pink "+" button in the "My Appointments" section of your dashboard to book a visit.

  • We currently only work with patients over the age of 18 at the time of their first visit.

    We may consider expanding to younger patients in the future, but that is not something we offer at this time.

  • Absolutely, ChroniusCare can act as your sole primary care provider, covering all aspects of traditional primary care, including preventative services.

    For patients with HMO or Medicaid plans that require an in-network PCP for referrals, we’ll work with your current in-network provider to ensure a smooth care process and can assist with finding a local PCP if needed.

  • No, you don’t need to leave your current primary care provider. You can choose to have Chronius Care as your main primary care provider or we can work alongside your existing provider, depending on what you prefer.

    We’re here to coordinate your care and collaborate with anyone on your healthcare team.

  • At our virtual-only primary care practice, we have the capabilities and tools to serve as your sole PCP, providing comprehensive care from the comfort of your home. While we adeptly handle most health concerns virtually, there are instances—like vaccine administration, specific physical exams, or treatments such as wound suturing—that necessitate in-person care.

    If that's the case, we will refer to a local urgent care, ER, or local provider, depending on the circumstance. Alternatively, if you have a preference for a local visit, we can help connect you to a nearby PCP and actively collaborate with them to ensure consistent and coordinated care. Our commitment is to ensure your healthcare is streamlined, friendly, and always tailored to your needs.

    Lastly, we require patients with Medicaid and certain HMO plans to have a local in-network PCP that we can collaborate with for orders. We can help you find this provider if you don't have one already.

  • As a Chronius patient you have access to: unlimited messaging with your interdisciplinary care team, virtual visits, robust care coordination, patient advocate services, and access to our patient empowerment platform. It also includes personalized care plans tailored to your individual needs.

  • At Chronius, our primary care providers are dedicated to guiding you through every step of your healthcare journey. If you don’t have a diagnosis, they work closely with you to help uncover the root cause of your symptoms. If you need a treatment plan, they’ll build one tailored to your needs, and if your symptoms change or you experience a flare, they’ll help you adjust your plan accordingly.

    They also ensure you have the right specialists in your care team, coordinating referrals and offering a well-rounded approach to managing your health. Beyond that, your provider is there to help you with preventive care, urgent issues, and navigating complex recommendations from other specialists. Think of them as your guide, advocate, and advisor—someone who is dedicated to your long-term well-being and overall health.

    Our care team will get back to you as soon as possible, usually within 24 hours, during regular business hours.

    If your consultation indicates the necessity for tests, our care coordination team will be on hand to help. They'll assist in scheduling your test, whether it's identifying a lab (we typically work with Quest) that accepts your insurance, arranging an in-home blood draw, or locating a nearby health service provider for more specialized testing with the earliest availability.

    Even though consultations are virtual, we prioritize ensuring that patients receive testing that is covered by their insurance and is as convenient as possible.

    Once we receive your test results, we'll promptly schedule a follow-up virtual consultation to discuss the findings and the next steps in your care.

  • Our practice is highly proactive when it comes to specialty testing. We believe in streamlining your care journey as efficiently as possible. To that end, if you have a specialist appointment on the horizon, we work closely with you to anticipate and complete the necessary tests your specialist might request.

    This approach ensures that, during the specialist visit, your doctor is equipped with all the information they need to take the next steps in your care without delay. Our primary goal is to ensure you receive timely and comprehensive care at every stage.

  • At Chronius, we're deeply committed to ensuring our patients have access to the treatments they need, especially when dealing with complex chronic conditions. Whether it's IV fluids, feeding tubes, or even cutting-edge experimental treatments, we're prepared to prescribe and oversee their management, just as any primary care provider would.

    Our approach is strictly grounded in evidence-based medicine; we will only prescribe treatments that have solid scientific backing. However, we're also always on the lookout for emerging treatment options, recognizing that the world of medical research moves rapidly.

    We don't just stop at prescribing. We understand that navigating insurance and specialty pharmacies can be a challenge. That's why we're here to advocate for you. If there are hurdles with insurance approvals, we'll step in to fight for your right to treatment. And when it comes to specialty pharmacies or specific requirements, our team will work alongside you to find the perfect fit and coordinate the logistics.   • Yes, our providers can prescribe medications as part of your care, just like any other primary care physician. Prescriptions are made at the provider’s discretion, based on what they believe is best for your health and treatment needs.

    Please note that Chronius does not prescribe controlled substances—such as painkillers (like oxycodone), anxiety medications (like Xanax), or stimulants (like Adderall).

    However, if you need medications that fall into the controlled substances category, we are more than happy to help connect you with specialists such as psychiatrists or pain management providers who are able to prescribe them, depending on your needs.

  • If you need to go to the hospital, we’re here to support you throughout the process. If your Chronius provider refers you to a specialist at the hospital, we’ll provide the referral and help with scheduling.

    If you visit the ER, we’ll make sure your medical records are transferred to Chronius so your provider can review your stay, test results, and any new treatments. We’ll also coordinate follow-up care and schedule an appointment to discuss your next steps.

    If you’re admitted, your Chronius care team will connect with the hospital and make sure you are advocated for during your stay. We’ll work directly with your clinical team to ensure continuity of care and that all aspects of your treatment are well-coordinated. After discharge, we’ll schedule an urgent visit with your Chronius provider to review any changes to your treatment plan and medications. We’ll also assist with any hospital bills or insurance issues to help ensure everything is handled smoothly.

  • Yes, we offer same- and next-day acute care visits with one of our nurse practitioners for conditions like urinary tract infections (UTIs), minor infections, rashes, coughs, cold and flu symptoms, and medication adjustments. However, for certain situations where a physical exam is needed—such as suspected fractures, severe cuts, sprains, breathing difficulties, chest pain, or deep infections—we may recommend visiting a local urgent care center to ensure you receive the appropriate in-person care.

  • At Chronius, every referral is personalized to your specific needs. Our care coordination team searches for providers who fit your preferences—whether that’s taking your insurance, being close to you, or having expertise in your particular conditions. We also ensure that the providers have a strong track record with other patients, so you can feel confident in your care.

    Each search is custom-tailored for every patient and referral, saving you hours of work and getting you to the right care faster. We refer to specialists, mental health professionals, nutritionists, physical therapists, home health services, infusion centers, and more. Plus, we keep a list of providers who haven’t met patient expectations to make sure you avoid them.

    We know how important it is to build a care team you can trust, and our approach ensures you’re connected with the best possible providers.

  • Yes, we can assist with medical forms, including accommodation letters, disability forms, handicap applications, FMLA paperwork, and more. We understand that these forms are often essential for our patients to access the support and accommodations they need.

    These services come with a small additional fee and may require a visit with your provider to ensure the forms are completed accurately and reflect your current health needs.

Membership & Payments

  • For our primary care visits, we are currently a self-pay practice. However, we are working hard to gain in-network status with as many payors a possible. Recently, some Chronius providers gained in-network status with most Aetna and Cigna commercial plans. While we continue to work hard on expanding our insurance coverage, every visit with us costs no more than the cost of an average co-pay.

    We do submit superbills on your behalf to your insurance company for potential out-of-network reimbursement.

    Moreover, our mental health therapy appointments are provided by partners who do take insurance. And all testing and specialist referrals will be prioritized to be within your insurance plan’s coverage.

  • Membership is $25/month when paid annually or $40/month when paid monthly, giving you access to deluxe care coordination, patient advocate support, and a suite of tools to manage your care online. Through our platform, you can schedule visits, message your care team, access and aggregate health records, and get personalized specialist referrals tailored to your conditions at any time.

    You pay for visits, medications, and labs only when you need them. Visits are self-pay at $40 per visit, regardless of insurance. Starting later this fall, if you’re covered by Aetna or Cigna, you’ll be able to pay us as an in-network provider for visits. You can also use your insurance to cover labs and medications.

    Your membership includes one 30-minute visit per month with a board-certified patient advocate. If needed, additional advocate visits are available at $80 per hour.

    We also offer receipts to submit for you to pay for your visits with your HSA/FSA as well.

  • At Chronius, having a full picture of your health is key to delivering the right care, especially for those managing complex conditions. Our providers go beyond what’s typical at most primary care practices by thoroughly reviewing your medical history to ensure we’re giving you the most personalized treatment. We’ve made it easy for you to share records and are proactive about gathering them to provide the best support possible.

    You can share any medical records with your Chronius care team, including clinical notes, lab results, testing reports, images, videos, and more. You can easily sync data from your outside patient portals, like MyChart, to automatically bring in all your records, or email them to us through our HIPAA-compliant email. If you don’t have your records, we can also request them on your behalf.

    Before your first visit, your provider will review your Health Profile (intake form) and will review as many records as possible, depending on the amount shared and timing of receipt. A more detailed review will happen between your first and second visits to get a fuller understanding after hearing your story firsthand.

    As an ongoing patient, your provider will review all specialist notes, lab results, and other shared records to stay up-to-date on your case and provide feedback when needed.

  • Yes, with your written permission, Chronius can send your records to outside parties and request records on your behalf.

    Sending records to outside parties

    You can easily send your medical records by emailing your Chronius care team or using your Chronius account to send a Chronius Health Summary. If you’d rather not handle it yourself, we’re happy to do it for you.

    Obtaining records from outside parties

    Sometimes, such as when you first join Chronius, we may need to get your medical records from other providers (like your previous doctor, specialist, or hospital). You can either sync your patient portals directly with your Chronius account to automatically share your data or sign a HIPAA release, allowing us to request the records on your behalf.

    For any specialist visits we refer you to, we’ll also request a copy of the clinical notes to ensure continuity of care.
